What is the CPHQ certification?

The CPHQ is the credential NAHQ describes as the only accredited certification in healthcare quality. It is accredited by the National Commission for Certifying Agencies (NCCA), and it shows that you have the knowledge expected of a competent healthcare quality professional: leading quality work, improving processes, measuring and analyzing data, keeping patients safe and meeting regulatory requirements.

People who take it work as quality and patient safety specialists, nurses in quality or accreditation roles, risk managers, data analysts, case managers and healthcare leaders. When you pass, NAHQ sends a digital badge (US candidates get it about two weeks after the exam), and from then on you can put “CPHQ” after your name.

Who can take the CPHQ exam?

Anyone can apply: there are no formal eligibility requirements. NAHQ recommends at least 2 years of experience in healthcare quality, because the questions are written for people who have done the work described in the content outline.

You apply and pay online at nahq.org. Once your application is processed, you have 90 days to schedule and sit the exam, so pick a date before you apply.

Exam format and what test day is like

The exam has 140 questions. Only 125 count toward your score; the other 15 are pretest questions NAHQ is trying out for future exams. They are mixed in at random and look exactly like the rest, so answer every question as if it counts.

Most of the exam is not recall. According to the candidate handbook, about 23% of the questions test recall of facts and concepts, 57% ask you to apply knowledge to a situation, and 20% ask you to analyze information. Expect scenarios: a dashboard to interpret, a team to set up, a survey finding to respond to.

A few practical rules from the handbook:

  • Navigation: you can move back and forth between questions and bookmark the ones you want to revisit before you submit.
  • Calculator: you don’t need one, but there is a calculator inside the testing platform. You can’t bring your own.
  • Breaks at a test center: allowed whenever you like, but the clock keeps running.
  • Breaks online: not allowed unless PSI approves a scheduled break as an accommodation in advance, and after that break you can’t go back to earlier questions.
  • ID at a test center: two current forms of identification, one with a photo, both with your name and signature.

The 7 CPHQ domains and how they are weighted

NAHQ’s content outline (effective March 15, 2023) splits the 125 scored questions across seven domains:

Domain Scored items Share of score What it covers
1. Quality Leadership and Integration 19 15.2% Strategic planning, quality governance, stakeholder engagement, culture of quality and safety
2. Performance and Process Improvement 27 21.6% QI methods (Lean, PDSA, Six Sigma), quality tools (fishbone diagram, FMEA, process map), teams, leading change
3. Population Health and Care Transitions 11 8.8% Population health data and strategies, health equity, social determinants of health, care transitions
4. Health Data Analytics 26 20.8% Data collection plans, measure selection, statistics, statistical process control, scorecards and dashboards
5. Patient Safety 18 14.4% Safety culture, event reporting, sentinel event review, root cause analysis, proactive risk assessment
6. Quality Review and Accountability 16 12.8% Compliance with guidelines and quality-based payment programs, practitioner performance evaluation, confidentiality of quality records
7. Regulatory and Accreditation 8 6.4% Accreditation options, statutory and regulatory requirements, compliance monitoring, survey readiness
Total 125 100%

Horizontal bar chart of the 125 scored CPHQ exam items by domain: process improvement 27, health data analytics 26, quality leadership 19, patient safety 18, quality review 16, population health 11, regulatory and accreditation 8 Chart: CPHQ Prep, from the NAHQ content outline effective March 15, 2023.

Two domains carry the exam: process improvement (27 items) and health data analytics (26 items) are 42% of your score. Regulatory and accreditation, by contrast, is only 8 questions. Weight your study time the same way.

How the CPHQ is scored

Your result is a scaled score from 200 to 800, and 600 is the passing score. It is not a percentage. NAHQ uses several versions of the exam, and scaling makes a score mean the same thing whichever version you get. The passing standard is set with the Angoff method: a panel of experts reviews every question to decide how many correct answers show the knowledge needed to pass. You are not graded on a curve, so other candidates’ results don’t affect yours.

You see your result on screen as soon as you finish, and it is saved in your NAHQ account. The report shows pass or fail, your scaled score, and the percentage of questions you answered correctly in each domain. The domain percentages are feedback only: your pass or fail decision comes from your overall score.

CPHQ pass rate

In 2023, 65% of US candidates passed the CPHQ exam, according to the NAHQ candidate handbook (version V.2024.7). That is the share of people who passed, not the score you need. Pass rates change from year to year, so check the current handbook if you want the latest figure.

Roughly one in three US candidates did not pass that year. With 77% of the questions testing application or analysis, the exam rewards applied judgment more than memorized definitions.

How much does the CPHQ exam cost?

The fee depends on your NAHQ membership tier and on whether you test in the United States or internationally. NAHQ members pay less than non-members, and paying by check costs a little more than paying online. A retake needs a new application and the full fee again, and recertification every two years has its own fee. Check the current amounts on NAHQ’s apply page before you budget, because NAHQ updates them.

If you don’t pass: retake rules

You can reapply 14 days after your exam date. You can take the exam up to three times in 365 days; after a third unsuccessful attempt you have to wait 365 days before testing again. Each attempt needs a new application and the full fee. NAHQ keeps the names of candidates who don’t pass confidential.

Use the domain percentages on your score report to decide what to study before the next attempt, keeping in mind that small domains (like regulatory and accreditation, with 8 items) give less reliable percentages.

Keeping your CPHQ: recertification

The CPHQ is valid for two years. Your first cycle starts on January 1 of the year after you pass. To renew, you need 30 continuing education (CE) hours during the cycle, tied to the current content outline, plus a recertification fee. CE has to be earned within the cycle, not during the grace period, and normal job duties don’t count.

How to prepare for the CPHQ exam

NAHQ says most candidates with healthcare quality experience spend six weeks to three months preparing. A plan that fits that window:

  1. Take a diagnostic first. Try our free CPHQ practice test: 14 questions, two from each domain, with an explanation for every answer. It shows which domains need the most work before you spend money on materials.
  2. Weight your time by the outline. Put about 40% of your hours into process improvement and data analytics, then leadership, patient safety and quality review. Don’t let regulatory details (8 items) eat a week.
  3. Practice application, not definitions. Since 77% of questions are application or analysis, spend most of your time on scenario questions and on explaining to yourself why each wrong option is wrong.
  4. Get comfortable with the numbers. The data analytics domain expects you to read control charts, choose the right measure and use basic statistics. Work real examples with the control chart calculator, the descriptive statistics calculator and the DPMO and sigma level calculator.
  5. Review weak domains, then rehearse. In the last two weeks, do long mixed question sets in one sitting to build stamina for three hours.

Individuals (XmR) control chart of 20 weekly ED door-to-provider times with mean 32.8 minutes, upper control limit 45.54 and lower control limit 20.06; week 14 at 47 minutes is above the upper limit Chart: CPHQ Prep, made with the free control chart calculator. Week 14 is a special-cause signal; every other week is common-cause variation.

If you study on your phone, CPHQ Prep has 249 exam-style questions across all seven domains, each with a written explanation, plus 93 key-term flashcards, an AI tutor that explains the reasoning behind each answer, a daily quiz and 200-question mock exams. It is free to download on iPhone and iPad.

One warning: stay away from “braindump” sites that claim to sell actual exam questions. Buying, sharing or reviewing materials that claim to contain real CPHQ questions is grounds for NAHQ to deny or revoke your certification.
